Please complete all required fields in this registration form accurately and thoroughly. This information is essential for participant safety, communication, and proper program organization.

Before submitting, carefully review each waiver and agreement listed below. These documents outline important policies, responsibilities, and permissions related to participation in Pacific Travel Ball.

To successfully complete your registration, you must check “AGREE” for each waiver section. Forms that are incomplete or missing required acknowledgments will not be processed or approved.

Pacific Travel Ball – Liability Waiver & Release of Claims

I, the undersigned parent/guardian of the registered participant, acknowledge that participation in Pacific Travel Ball activities, including but not limited to practices, games, camps, and events, involves inherent risks of injury, illness, or damage.

I understand and voluntarily assume all risks associated with participation, including but not limited to physical injury, contact with other participants, equipment-related hazards, and weather conditions.

On behalf of myself and my child, I hereby release, waive, and discharge Pacific Travel Ball, its coaches, staff, volunteers, affiliates, and facility providers from any and all liability, claims, demands, or causes of action arising out of or related to any injury, illness, loss, or damage incurred during participation.

I certify that my child is physically fit and able to participate in baseball activities.

By checking this box, I acknowledge that I have read, understand, and agree to this waiver.

Pacific Travel Ball – Medical Authorization Form

I, the undersigned parent/guardian, authorize Pacific Travel Ball staff, coaches, and representatives to obtain emergency medical treatment for my child if I cannot be reached in a timely manner.

I understand that every effort will be made to contact me prior to any treatment. However, in the event of an emergency, I give permission for my child to receive necessary medical care, including first aid, emergency transportation, and treatment by licensed medical professionals.

I agree to assume full responsibility for any and all medical expenses incurred.

I confirm that all medical information provided during registration is accurate to the best of my knowledge.

Pacific Travel Ball – Media Release Authorization

I grant permission to Pacific Travel Ball to use photographs, video recordings, and/or audio recordings of my child taken during participation in activities for promotional and marketing purposes.

This includes, but is not limited to:

  • Social media (Instagram, Facebook, etc.)

  • Website content

  • Advertising materials

  • Print and digital marketing

I understand that:

  • No compensation will be provided for use of these materials

  • My child’s name may or may not be used in connection with media

  • All media becomes the property of Pacific Travel Ball

If I do not wish for my child to be included in media, I will notify Pacific Travel Ball in writing.

Pacific Travel Ball – Code of Conduct

By participating in Pacific Travel Ball, both players and parents/guardians agree to uphold the following standards:

Player Expectations:

  • Show respect to coaches, teammates, opponents, and officials

  • Demonstrate good sportsmanship at all times

  • Follow instructions and maintain a positive attitude

  • Refrain from inappropriate language or behavior

Parent/Guardian Expectations:

  • Support all players, coaches, and officials respectfully

  • Avoid coaching from the sidelines unless invited

  • Promote a positive and encouraging environment

  • Address concerns privately and respectfully with staff

Pacific Travel Ball reserves the right to remove any participant or spectator who violates this code of conduct without refund.
